Tender Description:: 1. The Government of Ghana through the GHANA HEALTH SERVICE has received support from the Global Alliance for Vaccines and Immunization (GAVI) and wish to apply a portion of it towards the Construction of 25 New Medical Waste Incinerators with stores at selected sites for Ghana Health Service as follows:

LOT 1 - GHS/EPI/GAVI-FUND/NCT/WKS/001/2019
Construction of 8 New Medical Waste Incinerators with Stores in the Ashanti and Brong Ahafo Regions at following Locations/Sites:
1. Agogo Presby Hospital
2. St. Vincent Clinic
3. Asokore Mampong
4. Kodie
5. Banda Ahenkro Health Centre
6. Nkrankwanta Polyclinic
7. Municipal Hospital
8. Yeji RCH

LOT 2 -GHS/EPI/GAVI-FUND/NCT/WKS/002/2019
Construction of 7 New Medical Waste Incinerators with Stores in the Central and Western Regions at following locations;
1. Hemang Health Centre
2. Essuehyia
3. Kasoa
4. Adabokrom
5. Dadieso Health Centre
6. Manso Amenfi Health Centre
7. Mpohor Health Centre

LOT 3 - GHS/EPI/GAVI-FUND/NCT/WKS/003/2019
Construction of 10 New Medical Waste Incinerators with Stores in the Greater Accra, Eastern and Volta Regions at the following locations/Sites
1. Aburi
2. Adeiso
3. Coltar
4. Tease
5. Kpone Health Centre
6. Prampram Health Centre
7. Sege Health Centre
8. Battor Catholic Hospital
9. Chinderi Health Centre
10. Ve Golokwati Health Centre

Additional Information:: 4. Tenderers are required to submit the following statutory documents as part of their tender validity as at the date of opening:

1. Certificate of Incorporation/commence business
2. Public Procurement Authority (PPA) Registration Certificate or Receipt of payment for Registration Fee
3. Valid Ministry of Water Resources, Works & Housing Certification
4. Valid Tax Clearance Certificate
5. Valid SSNIT Clearance Certificate
6. Valid Labour Certificate
7. Other documents indicated in the Tender Data sheet

Tenders shall be valid for a period of 90 days after Tender opening and must be accompanied by a tender security of 2% of tender price in Ghana Cedis from a reputable Insurance Company.
