Tender Name:: Proposed Completion of Community Health-Based Planning Service (CHPS) Compound at Kato

Tender Description:: The Government of Ghana through the Ministry of Health of the Republic of Ghana has allocated funds towards the cost of Construction Works and intends to apply part of the funds to cover eligible payments under the Contracts for Proposed Construction of Community-Based Health Planning Service (CHPS) Compounds at Selected Locations as stated below;

Proposed Completion of Community Health-Based Planning Service (CHPS) Compound at Kato in the Berekum Municipality (LOT 005 – Bono Region).

Additional Information:: Tenders shall be valid for a period of 180 DAYS after Tender opening.

Tenders must be accompanied by a Tender security of 2% of the Tender price or its equivalent in a convertible currency for each Lot from a reputable Insurance Company or Bank.

Tenderers are required to submit the following statutory documents as part of their Tender, which must be valid as at the date of opening:

1. Valid Ministry of Water Resources, Works & Housing Certification D1, K1 or D2,K2
2. VAT Registration Certificate
3. Valid Tax Clearance Certificate
4. Valid SSNIT Clearance Certificate
5. Valid Labour Certificate
6. Certificate of Incorporation/Commence Business
7. Public Procurement Authority (PPA) Suppliers Registration Certificate
8. Other documents indicated in the Tender Data
